Fill out all the information to register.  The physical form can be uploaded to Family ID. Only sections 5 and 6 of the form need to be uploaded.  We encourage all to upload the form, but paper copies will be accepted. They should be sent to the Athletic Office.

The PIAA form is the only form accepted as a sports physical. Download the forms here: PIAA Physical Form

It must be dated AFTER May 1st, 2025.

Boys V/JV Basketball – 11/14

Boys 9^th Basketball – 11/14

Girls V/JV Basketball – 11/14

Swimming & Diving – 11/14

Unified Bocce – 11/17

Wrestling – 11/14

Middle School

Boys 7^th & 8^th Basketball – 11/17

Jr. High Wrestling – 11/17

Girls 7^th & 8^th Volleyball – 1/26*

*Registration for volleyball will open at a later date*

FamilyID will close on NOVEMBER 11, 2025 @10pm
 

November 11, 2025 - Last day to register on FamilyID and submit Section 6 (CIPPE) & for ALL HIGH SCHOOL TEAMS & ALL MIDDLE SCHOOL TEAMS.
